1)  Complete this form and sign (both signatures if a joint application).
2)  Attach a copy of your unexpired driver's license (both if a joint application).*
3)  Enclose a check or money order made payable to "The State Credit Union" in the amount of $7.00.

Please submit your completed application and all supporting documentation to the address indicated on the application or you can fax it to 304-558-0137. If you have any questions or require assistance with completing your application, please contact the Credit Union at 304-558-0566

*If the address listed on your driver's license does not match your physical address, please provide documentation of your current physical address (example would be a copy of a utility bill).

3)  If this is a name change request please provide proper documentation (example marriage certificate, divorce certificate).
4)  Removal of someone from the account requires proper documentation (example death certificate) and all applicable signatures would be required.

payroll deductions or to make changes to your payroll deductions.

3)  Submit to your Payroll Clerk for signature and processing.
